Affidavit of dependency

Family & estates

First check which process needs the affidavit. The SASSA form is for a Care Dependency Grant and records a caregiver’s relationship to a child who lives with them. A separate SARS medical-scheme audit may require an affidavit about whether the main member is a dependant. Use the section below that matches your request.

How to use it

  1. Check the care declaration The caregiver statement combines your role and the child’s residence. It names a parent, foster parent or primary caregiver, and says the child lives with that person. Read the full declaration so you can check both parts together.
  2. Review the child’s placement The form includes a separate declaration about an institution funded by the state. Check that wording alongside the care declaration; it addresses a different detail about the child’s circumstances.
  3. Complete the relevant personal sections The marital-status section applies if you are married, divorced or widowed. The spouse or partner document section names an ID document, decree of divorce and death certificate. It also asks you to explain if you do not have any of those documents.
  4. Read the income and oath wording If you or your spouse have no source of income, the affidavit asks how you currently survive. It also includes a declaration that the information is true and correct to the best of your knowledge, and a statement about taking the prescribed oath and its effect on your conscience.
